Jab se tujhe hai jaana = Since I have known you
Dilne kaha na mana = my heart refuses to follow my commands
sach to hai tu kahan main kahan = It is true where are you and where am I means there is a difference between our respective satus.

Jaana can mean going but here it means knowing. Also if they wanted to say 'where you are at, I am at' they would have said ' tu jahaan main wahaan'. Instead they have said tu kahaan main kahaan.
